RESPONSIBILITIES: I. PERSON CENTERED SERVICES/PROGRAMS
A. Implement/support objectives and goals as identified in the person’s Person Centered Support Plan (PCSP). Document and follow up, as needed.
- Promote independence by assisting persons in preparing their own lunches to the best of their ability (i.e., assist with heating up food, how to operate a microwave, assist person with clean up, etc.). Be knowledgeable about all person’s diets (i.e., diabetic diet, pureed diet, mechanical soft, etc.). For those that require total support, provide hands on assistance with persons when eating. Use adaptive equipment with those who require it to assist in their independence (i.e., lip plates, adapted utensils and cups.
- Support persons who require assistance with personal needs in the restroom at least twice a day and as needed – level of support provided may include but not limited to total support, hand over hand support, verbal prompts, etc. Staff are trained by the nurse to provide good peri-care.
- Assist persons with proper positioning and safety while sitting in a wheelchair.
- Assist persons with transfers to include the use of a Hoyer Lift and/or two-person transfer.
- Be aware of communication needs and challenges for individuals as they experience aging, behavioral, psychological, and/or medical issues.
- Using person’s PCSP and/or input from individuals and their teams, provide in-house activities that encourage independence, productivity, and inclusion. Use creativity to develop new activities for persons to try as well, to promote growth and new learning experiences.
- Using person’s PCSP and/or input from individuals and their teams, accompany persons on community outings and volunteer opportunities. Assist persons in obtaining their maximum potential while in the community, encouraging independence, productivity, inclusion, and integration.
B. Serve as a positive role model for all persons supported using principles of Positive Behavior Supports (PBS)
C. Creatively assist in the planning and organization of daily, weekly, and monthly activity schedules.
D. Collaborate with the Program Coordinator and team to create, plan, and implement additional opportunities in the community.
E. Adhere to and be knowledgeable of all safety rules and regulations pertaining to transportation, building, and the well-being of all.
F. Adhere to program schedule keeping in mind the routine may change due to health, aging and mental health issues.
G. Communicate with the Program Coordinator prior to contacting parents, guardians, residential providers, and Case Managers regarding issues, needs and/or concerns.
H. Provide rotation of attention to all persons so that they feel acknowledged and supported. Be mindful how words, tone, pitch, and body positioning are used as they may be misperceived by others – sense of humor must be appropriate for the workplace.
I. Provide emotional support to persons supported and co-workers as they experience loss and change in relation to the aging process.
II. DOCUMENTATION
- Maintain accurate, thorough, and efficient documentation in accordance with TARC, Inc. Policies and Procedures:
- Complete division and program specific paperwork in a timely manner.
III. TEAM CONCEPT
- Convey a positive image of TARC, Inc. and its program to the community.
- Participate in team meetings to identify/advocate/support persons supported’s wants, needs, goals and dreams.
- Continue communication with persons supported and support team to generate opportunities and ideas, which will support goals and dreams to persons supported’s satisfaction and independence.
- Develop and maintain an effective, professional relationship with team, parents, guardians, colleagues to ensure effective communication regarding universal service delivery and satisfaction of services.
- Actively participate in all team meetings through effective communication, open to suggestions, flexibility, and new ideas.
IV. OTHER DUTIES
- Provide input for needed materials and equipment necessary for meeting persons supported needs.
- Assist in the upkeep, organization, sanitation and maintenance of the program environment and any materials/equipment/vehicles utilized.
